Hi,
I've never homeschooled before and was not able to get any assistance from the school in our neighborhood, so I bought a book on my own and have been using it to provide lessons.  It is titled:  Grade 2 Learn at Home. 
My first question is simple, I have to submit a lesson plan and a quarterly report.  Do you think it would be acceptable for me to copy the lesson plans from my book and submit it as an attachment or would I have to retype it to fit the form they provide?  My lesson plans are broken out first daily and then weekly.  I don't really see the necessity in reworking/rewording or retyping the lesson plans to fit the form.  However, if I have to, I will. 
My second question is I have to submit a quarterly report and I have no clue what it should look like on the form or the total number of hours that I should be listing for each quarter.
The form is broken out into 4 columns:  Subject, Description of Material Covered, Hours of Instruction, Grade or Written Evaluation.
Any assistance would be much appreciated.
 
I would call the office to which you must submit the form and ask them for examples, then adapt them to reflect what you are doing at home.  My state is very flexible, but all states are different.
http://www.hslda.org --- there are many different requirements depending on your state. the HSLDA is a great place to start to gather info.
